Abstract

The increasing prevalence of chronic diseases such as diabetes, cardiovascular disorders, obesity, and hypertension has necessitated the development of innovative, scalable, and personalized healthcare solutions. Personalized nutrition, supported by artificial intelligence (AI) and telemonitoring systems, has emerged as a transformative approach to managing chronic conditions through tailored dietary interventions. This paper presents a comprehensive analysis of AI-driven diet optimization frameworks integrated with telemonitoring technologies. It explores how machine learning algorithms, wearable devices, and real-time data analytics enable dynamic and individualized dietary recommendations based on physiological, behavioral, and environmental factors.

The study examines key components of personalized nutrition systems, including data acquisition from biosensors, dietary intake tracking, metabolic profiling, and predictive modeling. Advanced AI techniques such as deep learning, reinforcement learning, and recommendation systems are analyzed for their role in optimizing dietary plans. Furthermore, telemonitoring platforms facilitate continuous patient engagement, adherence tracking, and remote clinical supervision, improving health outcomes and reducing healthcare costs. Despite significant advancements, challenges remain in data integration, privacy, model interpretability, and clinical validation. This paper critically evaluates these issues and proposes future directions, including multimodal data fusion, explainable AI, and integration with digital health ecosystems. The   findings highlight the potential of AI-driven personalized nutrition systems to revolutionize chronic disease management through precision, scalability, and accessibility.
