Abstract

The rapid expansion of digital health ecosystems and telemonitoring platforms has revolutionised patient care, offering real-time data access, continuous monitoring, and enhanced diagnostic efficiency. However, this technological innovation has simultaneously introduced critical cybersecurity vulnerabilities, creating significant risks related to data breach, identity theft, system manipulation, device compromise, ransomware attacks, and operational disruption. This research paper examines the cybersecurity landscape surrounding telemonitoring systems, specifically analysing risk vectors, threat actors, system-level vulnerabilities, and organisational weaknesses. By applying recognised cybersecurity frameworks, including NIST, ISO/IEC 27001, and Zero-Trust architecture, the study evaluates the adequacy of existing mitigation strategies and proposes a comprehensive, multi-layered security model tailored for digital health environments. Through case studies from India, the United States, Europe, and Asia-Pacific, the research highlights real-world breaches and their consequences, emphasising the urgent need for robust governance, incident response systems, cybersecurity audits, encryption protocols, and staff training. The findings demonstrate that telemonitoring systems require continuous risk assessment, proactive threat intelligence integration, and adaptive security mechanisms to safeguard sensitive health data and ensure trust in modern digital health infrastructures.
